Every year, some hospitals and insurance plans end their contracts. It is a business negotiation between two large organizations, and the people most affected are the patients in the middle. If it happens to your plan, it does not mean you did anything wrong, and it does not mean you are out of options.
If your hospital or doctor leaves your plan's network, your plan still exists and your coverage continues. What changes is the cost of using that particular hospital or doctor: out-of-network care usually costs more, and with some plan types it may not be covered at all except in emergencies. Emergency care is always covered at the nearest hospital regardless of network.
The main window is the Annual Enrollment Period each fall, October 15 through December 7, when anyone with Medicare can switch plans for a January 1 start. Depending on your situation, there can be other windows during the year. An agent can check which ones apply to you, honestly and for free.
The question is not "which plan is best" in general. It is which plans in your county include your doctors and your hospital, cover your prescriptions, and fit your budget. That is a checkable list, not a sales pitch.
